IMPORTANT:

• Please be aware that many insurance companies require patients to have a REFERRAL on file before meeting with a specialist.  It is your responsibility to check with your insurance carrier and make sure you have a referral before your appointment (if needed).

• Please bring your INSURANCE CARD, we need to update it every time you see us. Note: we are not able to scan your card off your phone.

• If you have had a BIOPSY performed in advance of a minor procedure (such as evaluation or removal of a skin lesion)  please let the office know ahead of time so we can obtain those results prior to your visit.

• If you have had any PRIOR IMAGING, please obtain that imaging on a disc and bring it with you to your appointment.  This is not necessary if your imaging was performed by Radiology Associates of Hartford, as Dr. Tandon has access to those images.
